Customize new phone ticket

Hello,
I wish customize my new phone tickets.
I am understandable, when I to use(wear out) create a new ticket, I want that in the text field, there are already fields by défault and my customer would not have more than has to inform these fields.

freetext fields, sla´s, ticket types, and customize file AgentTicketPhone.dtl and AgentTicketEmail.dtl
