OTRS 3.2.2 Packages are not installed !!!

An error occurs when installing modules (packages):

But after coming back into the system the package is miraculously work and not see it in the list of installed packages.

System configuration: OTRS 3.2.2: Application Server on Ubuntu Server 12, database server on MsSQL 2012

Surely no one knows what is the cause of the error?

local repository does not show any installed package!
