Process ticket attachment issue

Hi all,
we have found an issue. Creating new ticket I choose some process, populate required information and want to add an attachment. Adding attachment this happens, interface of new ticket looks different without a header. Also, Service field gets blank with no value chosen. If this ticket is submitted everything is fine and attachment is added.

Does anyone have some problem or any suggestion?
