survey model feedback form ticket number

Hi ,

we are using survey model.

1 created General Info details
2 Questions created
3 Survey model new to activated
4 now once ticket closed survey form link information got mail email.


customer after receiving the email survey link. once click link feedback form show question. there is no ticket number and other detail.

how can add ticket details or link of ticket number???
