Closing Ticket Issue

I am running OTRS 3.0.8 and just today have had a closing ticket issue brought to my attention. I thought initially that it was most likely due to the ticket being locked or the agent not being the owner of the ticket. However upon further investigation, I found that after hitting submit after trying to close the ticket, the close ticket window would stay up and become blank. I would have to close the window manually. In the ticket history, it shows the ticket as being closed but the status indicates that the ticket is still open. The only way I could close the ticket was by using the generic agent. Does anyone have any ideas on why this could be happening or has anyone else encountered this issue. I have been using 3.0.x for a good while and have yet to see this happen. Also, would I be able to find more information in the system log, it seems to be not turned on under the admin tab where can I find the actual log in the file structure?
