Adding Custom Fields

I am looking for a way to add custom fields to the ticket screen. Can anyone add some insight on how this can be done?

Also, has anyone found a way to add a subject tree or categorization to these tickets?

You can use your own web form and send all the other info fields as part of the body of an email or SOAP to the the ticket engine.
OTRS 3.3.x (private/testing/public) on Linux with MySQL database. Also on github.
