Problem with the assignation of an agent.

Hi friends, thanks for your attention, I have a problem with my OTRS 5 in CEntos 6, the problem is that when I assigning a responsible to a ticket, normally in the header of the assignment is shown

From: Supervisory"
To: "Agent Assigned"
Subject:-----

But in my first attempt of assignment only it shows:

Supervisory"
Subject:-----

But in the second attempt , it shows the correct content. What could be the inconvenience ?.
From: Supervisory"
To: "Agent Assigned"
Subject:-----
